EAS User Authentication - LDAP Configuration Form

This form needs to be filled out and returned if clients wish to utilize LDAP(S) to perform their user authentication.  Note that if your LDAP server is not accessible over the public internet, a VPN tunnel will need to be created.  In this case the VPN configuration form will also need to be filled out and returned.
